Daily Symptom Tracking for COVID-19 Screening
All members of the Radford University community are asked to monitor their health daily. This tool is intended to assist with the consistency of your regular health screening. Setting a daily routine and always responding to these questions first thing in the morning is a good approach to keeping yourself and others safe and healthy.
All Radford University students, attending in-person classes or participating in a clinical or other field experience or internship are expected to complete this symptom tracking tool on a daily basis. Faculty and staff are also required to complete this tool daily.
If you have questions about COVID-19 and the University's plan for Fall 2020 Early Opening, please visit the Campus Reopening website for information. You can also contact COVID19@radford.edu. Questions regarding medical care should be directed to a healthcare provider or Student Health Center if you are a student.
The information collected from the COVID-19 Daily Symptom Tracking will be used to connect individuals with recommended resources based on symptoms. Information will only be shared with the response teams working to reduce the impact of the COVID-19 virus.
For technical problems submitting this form, please contact the Radford University Technology Assistance Center at www.radford.edu/itonestop or 540-831-7500.
This survey tool was adapted from COVID Health Check-ME system developed by a team at the College of Health and Human Services at George Mason University.
